At the time of your initial appointment please bring:
- Photo ID
- Insurance cards
- A valid referral
- CO-Pay
- A fully completed patient information sheets
- Any x-rays, CTs, MRIs (actual films and reports), if not performed by North Idaho Imaging or Inland Imaging.
- Exact dates of injury
At the time of follow-up visits please bring:
- Insurance cards
- CO-pays
- Updated referrals
- Any studies
- An updated patient information sheet every 6 months
CO-Pays, referrals and current billing information are required prior to service or your appointment will be rescheduled to a time when they are available. It is your responsibility to bring these to the office. All information on the patient information sheets, especially injury information, insurance information, and your identifying information must be accurate. Knowingly falsifying any of this information constitutes fraud and will result in dismissal from the practice.
Prescription Refill - Narcotics and other prescriptions will be prescribed by Dr. Larson in the immediate post-operative period only (usually no more than 8 weeks after surgery). Chronic pain requiring more protracted courses of narcotics is best-managed by a specialist trained to deal with chronic pain.
Narcotics will not (under any circumstances) be reordered during weekend
or night on call hours.
On Call - After hours care is available for acute and urgent problems. Our Neurosurgery coverage team of doctors, physician assistants, and nurse practitioners will be available on a rotating basis to answer after-hour calls. A Neurosurgeon will be available as necessary for medical and surgical emergencies.
Medication refills will not be done after hours, especially of narcotics. The on call health care provider must be able to speak to the patient himself/herself, and not to an intermediary to respect privacy concerns.
Disability - Patients on disability at the time of their initial evaluation will need to have further forms completed by the provider who put them on disability initially.
Only short-term (peri-operative) disability will be covered by this office, except for catastrophic disability.
Payments - We request that fees for office visits be paid at the time service is rendered. At that time, we'll provide you with a receipt that you can attach to your insurance form for submission. For larger amounts such as surgical fees, our office manager will work with you to arrange a payment plan that fits your budget. CO-Pays are due when you present to the office for an appointment.
Insurance - Our office will file your insurance claims for major procedures. However you maintain financial responsibility for all services rendered. If you have any questions about reimbursement we are always available for consultation.
