Office Hours - CDA Spine is open Monday through Friday from 8:00 AM to 5:00 PM. If you wish to schedule an appointment, please contact us at (208) 765-9100

At the time of your initial appointment please bring:

  1. Photo ID
  2. Insurance cards
  3. CO-Pay
  4. A fully completed patient information sheets
  5. Any x-rays, CTs, MRIs (actual films and reports), if not performed by North Idaho Imaging or Inland Imaging.
  6. Exact dates of injury if applicable
  7. Workers Compensation Claims (If applicable): please bring claim number, claims manager name, and claims manager contact information including phone number, fax number, and mailing address
  8. Motor Vehicle Accident (If applicable) please bring current PIP balance, ledger balance statement, and name and contact info of attorney


At the time of follow-up visits please bring:

  1. Insurance cards
  2. CO-pays
  3. Updated referrals
  4. Any studies
  5. An updated patient information sheet every 6 months

, referrals and current billing information are required prior to service or your appointment will be rescheduled to a time when they are available. It is your responsibility to bring these to the office. All information on the patient information sheets, especially injury information, insurance information, and your identifying information must be accurate. Knowingly falsifying any of this information constitutes fraud and will result in dismissal from the practice.

Prescription Refill - Narcotics and other prescriptions will be prescribed by CDA Spine clinicians in the immediate post-operative period, generally 6-12 weeks depending on the procedure. Chronic pain requiring more protracted courses of narcotics is best-managed by your primary care provider or a specialist trained to deal with chronic pain.  If you feel that your circumstance requires special attention please discuss the situation with our clinicians prior to your procedure.  If you should require prescription refills, contact your pharmacy during regular business hours.  Please plan ahead and allow up to 24 hours turnaround time for requests to be filled.

Narcotics will not (under any circumstances) be reordered during weekend
or night on call hours.

On Call - After hours care is available for acute and urgent problems. Our Neurosurgery coverage team of doctors, physician assistants, and nurse practitioners will be available on a rotating basis to answer after-hour calls. A Neurosurgeon will be available as necessary for medical and surgical emergencies.  Medication refills will not be done after hours, especially of narcotics. The on call health care provider must be able to speak to the patient himself/herself, and not to an intermediary to respect privacy concerns.

Disability - Patients on disability at the time of their initial evaluation will need to have further forms completed by the provider who put them on disability initially.  Only short-term (peri-operative) disability will be covered by this office, except for catastrophic disability.

Payments - We request that co-pays and any deductible/co-insurance fees be paid at the time service is rendered. At that time, we'll provide you with a receipt that you can attach to your insurance form for submission. For larger amounts such as surgical fees, our billing department will work with you to arrange a payment plan that fits your budget. Co-Pays are due when you present to the office for an appointment.

Insurance - As a courtesy to our patients, both in-network and out-of-network, our office will file your insurance claims for all procedures. However you maintain financial responsibility for all services rendered. If you have any questions about reimbursement we are always available for consultation.
